Home Modifications Service
The Home Modification Service is funded to provide services to frail aged and younger people with disabilities, and their carers. Charleville Home Modifications Service receives funding through the Commonwealth Home Support Program and Queensland Community Care.
The Home Modifications Service's purpose is to prevent the premature or inappropriate long term institutionalisation of clients who have been assessed as being eligible, by provision of physical and structural modifications to their principal place of residence. Clients would include frail or ‘At Risk’ aged persons, young/old, moderately or severely disabled or the designated carer for an eligible client. The Home Modifications Service provides alterations to homes of eligible clients; e.g. installing ramps and stair climbers for home access, and modifying bathrooms for the safety and comfort of daily bathing routines.
Brendan Albury is the project officer for the Home Modifications Service. In consultation with Occupational Therapists and tradespersons, Brendan's role is to provide clients with the options they have available, prepare plans and complete major modifications, provide service assessments and determine eligibility.
The Charleville Home Modifications Service can provide financial assistance to eligible clients of 50% of the total costs for most modifications. For clients unable to meet the full amount of the required client contribution, payment options can be negotiated with the Service and can be directly debited on a fortnightly basis through Centrelink.
Ramps and stair climbers are owned by the Service and are leased to the client. The lease payment also includes installation costs, and covers all maintenance and removal expenses when the service is no longer required.
To be eligible for a service you must:

Be over 65 years of age; or

Have a moderate, severe or profound disability; or
-
Be caring for someone with the disability.
People with a disability who already receive an equivalent service through another program are not eligible to receive a CHSP service.
